We are required to keep records of all patients we treat. This includes medical information, for example, medication, medical history, assessment and treatment records etc, along with identifying and demographic information (name, age, address etc).
All enquiries, personal information and records will be treated in the strictest confidence, in accordance with current GDPR and CSP guidelines. Records will be held on-file in a secure web-based database compliant with data handling and medicolegal standards. Client information and records will not be shared with any third party without prior written consent. Any supporting data or images captured will not be used for promotion or advertising purposes without prior consent. Your right to be forgotten cannot override the requirement for medical notes to be held for the minimum period as dictated by governing bodies.
Permission from a parent or guardian is required when assessing or treating patients under the age of 16 years. When assessing or treating adults with reduced capacity, document AWI (section 47) must be presented before treatment commences.
